PCMH Course Curriculum

Target audience:

These courses are designed for all levels of staff at health care centers who are interested in having his/her organization becoming recognized as a Patient Centered Medical Home.


The PCDC / Essential Learning Patient Centered Medical Home five (5) course curriculum will walk your staff through the standards and guidelines.  Through the use of interactive lessons and experience gained around implementation of the change concepts, you will be able to stimulate your own ideas for implementation or adaptation of these concepts at your organization.

This curriculum begins with with an overview of the key tenets of the NCQA PCMH 2011 Standards and Guidelines.  Learn field-tested strategies to successfully prepare for PCMH recognition through team selection, communication with key stakeholders, and completion of an assessment of your organization.

Interactive lessons and your guided PCMH assessment will assist you in deciding on a road map to achieve transformation or recognition that best suits your organization.

As you move through each of the PCMH standards, you will learn to:

  • apply best practices
  • identify gaps in & reorganize your processes
  • determine a planned approach for managing & coordinating care
  • improve & measure your performance
  • identify the connection between all of these processes andenhancements in patient care


You'll finish the curriculum by revisiting, in more detail, some of the topics that are critical to a Patient Centered Medical Home (PCMH) transformation. "The Change Concepts for Practice Transformation" will address some of the issues health care organization's struggle with as they move towards becoming a Patient Centered Medical Home. Completing this curriculum will be a significant step in the transformation of your practice to a Patient Centered Medical Home.
